In the competitive divisions (10U and older), coaches must provide a Game Card to the referee before the game is permitted to start. The Referee will use the card to track substitutions, score, injuries, cautions and any other information pertinent to the game.
The Game Card must be completed in advance of the game and contain details of the time and location for the match.
It must also contain the names of the players, in order of jersey number. And the name of the coach and of your opponent's coach.

After filling in the card, both sides must be printed. You must print onto card stock.
Here is an example of what a correctly completed Game Card should look like before handing it to the referee prior to kickoff.

At the end of the game, the the coach should always, always, always, ensure that they submit the game card according to the instructions provide by their Division Coordinator. If you are unsure, please contact your Division Coordinator for clarification. The process may vary from division to division, and field to field.
